 Senior/Elder Care Services

 Geriatric Assessment
Review physical, mental, social, and environmental issues to develop a plan of care. This assessment is conducted by a registered nurse and is the basis for determining the level of care needed and all other work to be completed.
Individualized Care Plan
After the geriatric assessment and financial assessment are completed, a blueprint can be developed to meet each individual’s needs. The care plan helps everyone remain focused. It balances independence with necessary care.
Assistance with Benefit Programs
Answer questions and counsel clients concerning benefit programs such as Medicare, Medicaid, and Veterans Administration, commercial health insurance and long term care insurance. This process evaluation helps determine available programs and services.
Nursing Home Placement
We assist with tours and applications and work to find the most appropriate setting that meets the individual’s needs.
PRI/Screen
Perform a New York State required assessment to determine level of care and eligibility for nursing home placement. This must be completed by a certified RN. When clients are moving from out of state back to New York, an assessment can be completed over the telephone in conjunction with the client’s physician.
Home Care Needs Assessment
When it is determined that services are needed in the home, our staff can arrange for services to be provided and monitor these services on an on-going basis. Services could range from personal emergency response systems, home delivered meals and non-medical support services to skilled nursing, therapies, and home health aide services.
